Abstract Objective: The present prospective clinical study aimed to investigate the effects of rapid maxillary expansion on the airway, correlating airway volumes obtained on multi-slice computed tomography and polysomnography assessment of oxygen saturation and apnea/hypopnea index. Methods: Twenty-four patients (11 with obstructive sleep apnea and 13 with residual snoring, mean age 10.0 (1.8), were enrolled in the study. Each patient underwent multislice computed tomography and nocturnal polysomnography before rapid maxillary expansion and after removal of maxillary expansion after six months. Airway regions were segmented, and volumes were computed. Results: The increase in oropharyngeal volume was significant in both groups. Oxygen saturation and apnea/hypopnea index were not statistically significant. No correlation was found between total airway volume, oxygen saturation, and apnea/hypopnea index changes between the time points examined. Conclusions: This study showed that when rapid maxillary expansion is performed in individuals with sleep-disordered breathing, there were statistically significant differences in oropharyngeal volume between pre- and post-rapid maxillary expansion, but there was no correlation between oxygen saturation values and oropharyngeal volume increase. Level of evidence: The article is classified as Evidence Level 3 (Three).

ABSTRACT

Introduction: In recent years, rapid maxillary expansion (RME) in prepubertal growth stages has been deeply investigated; however, its study has recently been increasing in adults without need for surgery. The objective of this review is to present the results reported in the scientific literature about RME in adults. Material and Methods: Six databases were used to carry out the secondary search of the review. The search was performed virtually and studies from the last six years were considered, with a last search performed until November, 2022. Results: A total of 253 studies were found, from which 20 studies were finally selected. RME in adults can result in substantial expansion similar to that obtained in patients who have not yet completed the ossification of the mid-palatal suture. Conclusion: The reviewed scientific literature shows evidence that there are various devices supported by mini-implants as non-surgical treatment options to correct transverse deficiency and help expand the maxilla in adult patients.


Introducción: En los últimos años se ha investigado profundamente la expansión maxilar rápida (ERM) en etapas de crecimiento prepuberal; sin embargo, recientemente se ha ido incrementando su estudio en adultos sin necesidad de cirugía. El objetivo de esta revisión es presentar los resultados reportados en la literatura científica sobre ERM en adultos. Material y Métodos: Se utilizaron seis bases de datos para realizar la búsqueda secundaria de la revisión. La búsqueda se realizó de manera virtual y se consideraron estudios de los últimos seis años, con una última búsqueda realizada hasta noviembre de 2022. Resultados: Se encontraron un total de 253 estudios, de los cuales finalmente se seleccionaron 20 estudios. La ERM en adultos puede dar como resultado una expansión sustancial similar a la obtenida en pacientes que aún no han completado la osificación de la sutura palatina media. Conclusión: La literatura científica revisada muestra evidencia que existen diversos dispositivos apoyados en mini-implantes como opciones de tratamiento no quirúrgico para corregir la deficiencia transversal y ayudar a generar disyunción maxilar en pacientes adultos.

ABSTRACT

Rapid maxillary expansion (RME) is the protocol of choice for early treatment of transverse malocclusions and it is achieved with fixed expanders that produce heavy forces to achieve midpalatal suture opening. One third of patients with maxillary constriction have a greater transversal deficiency at the intercanine width than at the intermolar. The aim of this article is to present a protocol for early treatment of posterior crossbite with the use of the expander with differential opening (EDO) for RME. It is a new appliance to perform RME and its main advantage would be to allow the expansion individualization. EDO was proposed aiming to promote greater expansion on the anterior rather than on the posterior region. A 9 year-old female sought treatment. The interceptive treatment plan was based on using EDO for RME. The post-expansion orthopedic response showed an opening of 7.5 mm between the maxillary central incisors, at the level of the incisal edge. It was observed an opening of 4.8mm in the midpalatal suture. The opening at a distance of 10 mm and 20 mm from the crest to posterior at the midpalatal suture were 3.9mm and 2.8mm. The upper intercanine distance showed an increase of 9.31 mm. The upper intermolar distance had increased 8.04 mm. The upper arch perimeter showed a difference from 74.02 mm to 80.11 mm . And the upper arch length, from 29.83 mm to 31.56 mm. The posterior crossbite was 2 mm overcorrected. Early diagnosis and treatment of posterior crossbite has a very favorable prognosis. (AU)


A expansão rápida da maxila (ERM) é a técnica de escolha para o tratamento precoce das más oclusões transversais e é ativada com expansores fixos que produzem forças pesadas para possibilitar a abertura da sutura palatina mediana. Um terço dos pacientes com atresia maxilar apresenta uma maior deficiência transversal na região intercaninos do que na região intermolares. O objetivo desse artigo é apresentar um protocolo de tratamento precoce para a mordida cruzada posterior utilizando o expansor maxilar Diferencial (EMD) para realizar a ERM. EMD é um novo dispositivo que pode ser empregado para realização da ERM e sua principal vantagem seria permitir a individualização da expansão. EMD foi proposto para proporcionar maior expansão na região anterior do que na região posterior da maxila. Uma menina de 9 anos de idade buscou tratamento. O plano de tratamento interceptativo proposto foi ERM utilizando o dispositivo EMD. A resposta ortopédica pós expansão mostrou uma abertura de 7,5 mm entre os incisivos centrais superiores, ao nível da borda incisal. Foi observada uma abertura de 4,8 mm na sutura palatina mediana. A abertura a uma distância de 10 mm e 20 mm da crista para posterior na sutura palatina mediana foi de 3,9 mm e 2,8 mm. A distância intercaninos superior apresentou aumento de 9,31 mm. A distância intermolares superiores aumentou 8,04 mm. O perímetro do arco superior apresentou diferença de 74,02 mm para 80,11 mm. E o comprimento do arco superior, de 29,83 mm para 31,56 mm. A mordida cruzada posterior foi sobrecorrigida em 2 mm. O diagnóstico precoce e o tratamento da mordida cruzada posterior tem um prognóstico muito favorável. (AU)

ABSTRACT Objective: The aim of this study was to compare the occlusal changes of rapid maxillary expansion (RME) and slow maxillary expansion (SME) in patients with unilateral complete cleft lip and palate (UCLP), by means of digital dental models. Methods: Group RME was composed by 22 patients (13 males and 9 females), with mean age of 9.9 years, treated with rapid maxillary expansion with Hyrax appliance in Center 1. Group SME was composed by 29 patients (14 females and 15 males), with mean age of 10.7 years, treated with slow maxillary expansion with quad-helix appliance in Center 2. Digital dental models of the maxillary dental arch were obtained immediately pre-expansion (T1) and 6-month post-expansion (T2). Transversal distances, arch perimeter, arch length, palatal depth, palatal volume and posterior tooth inclination were digitally measured. Interphase and intergroup comparisons were performed with paired t-test and independent t-test, respectively. Results: Intercanine expansion was 4 to 5mm in both groups, and increase in the intercanine distance was similar for both groups. RME group showed a greater increase in arch distances at the region of permanent premolar and molars, compared to SME group. Arch perimeter increase was greater for RME group, compared to SME. No differences were found for arch length, palatal depth, palatal volume and posterior tooth buccal tipping. Conclusion: RME and SME produced similar dentoalveolar outcomes, with greater amount of expansion on RME group.


RESUMO Objetivo: O objetivo do presente estudo foi comparar, por meio de modelos digitais, as alterações oclusais após a expansão rápida da maxila (ERM) e após a expansão lenta da maxila (ELM) em pacientes com fissura labiopalatina completa unilateral (FLPU). Métodos: O grupo ERM foi composto por 22 pacientes (13 homens e 9 mulheres), com idade média de 9,9 anos, tratados com expansão rápida da maxila com aparelho Hyrax no Centro 1. O grupo ELM foi composto por 29 pacientes (14 mulheres e 15 homens), com idade média de 10,7 anos, tratados com expansão lenta da maxila com aparelho quadri-hélice no Centro 2. Modelos digitais da arcada dentária superior foram obtidos imediatamente pré-expansão (T1) e seis meses pós-expansão (T2). Distâncias transversais, perímetro da arcada, comprimento da arcada, profundidade do palato, volume do palato e inclinação dos dentes posteriores foram medidos digitalmente. As comparações interfases e intergrupos foram realizadas com teste t pareado e teste t independente, respectivamente. Resultados: A expansão da arcada na região dos caninos foi de 4 a 5 mm em ambos os grupos, e o aumento na distância intercaninos foi semelhante nos dois grupos. O grupo ERM apresentou maior aumento nas distâncias entre pré-molares e molares permanentes, em comparação ao grupo ELM. O aumento do perímetro da arcada foi maior para o grupo ERM, em relação ao ELM. Não foram encontradas diferenças para comprimento da arcada, profundidade do palato, volume do palato e inclinação vestibular dos dentes posteriores. Conclusão: ERM e ELM produziram resultados dentoalveolares semelhantes, com maior quantidade de expansão no grupo ERM.

Resumen Objetivo: Describir los resultados obtenidos con MARPE en pacientes adultos jóvenes en relación al ancho transversal intermolar, ancho transversal de cavidad nasal, complicaciones y otros resultados informados. Método: Se realizó un análisis siguiendo la guía Prisma, utilizando bases de datos Pubmed, SCIELO, Science Direct, Scopus, Sistema de información sobre literatura gris en Europa, Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Google Académico, además de una búsqueda manual de libros y artículos científicos. Aplicando los criterios de inclusión y exclusión se analizaron 9 estudios. Resultados: MARPE aumenta el ancho transversal intermolar y el ancho de la cavidad nasal en pacientes adultos jóvenes. Las complicaciones periodontales y dentoalveolares registradas no son concluyentes. Conclusión: MARPE puede ser un tratamiento clínicamente viable y efectivo para pacientes adultos jóvenes que presenten deficiencia transversal maxilar.

Abstract Objective: To describe the results obtained with MARPE in young adults regarding intermolar width, the transverse width of the nasal cavity, complications, and other results. Method: We conducted a literature review following Prisma guidelines in Pubmed, SCIELO, Science Direct, Scopus, the System for Information on Grey Literature in Europe, Latin American and Caribbean literature in Health Sciences, and Google Academic. We also conducted a manual search of books and scientific papers. The full articles were analyzed, and nine articles were selected after applying the inclusion and exclusion criteria. Results: The literature indicates that MARPE increases transverse intermolar width and the transverse width of the nasal cavity. The periodontal and dentoalveolar complications reported are inconclusive. Conclusion: MARPE could be a clinically viable and effective treatment for young adults with transverse maxillary deficiency.

ABSTRACT Objective: The aim of this study was to test the null hypothesis that there is no difference at the midpalatal suture opening after rapid maxillary expansion (RME) using Expander with Differential Opening (EDO), Hyrax-type and Haas-type expanders. Methods: Occlusal radiographs of 52 patients (19 males and 33 females; average age= 9.46?1.20 years) treated with RME were divided into three groups, according to the expander used: EDO (n=17), Hyrax-type (n=21) and Haas-type (n=14). The evaluated variables were: A) Distance between the maxillary central incisors at the incisal edge; B) Distance between the alveolar ridges at the midpalatal suture; C) Suture opening at 10-mm distance from the crest to posterior, at the midpalatal suture; D) Suture opening at 20-mm distance from the crest to posterior, at the midpalatal suture; and E) Suture opening at 30-mm distance from the crest to posterior, at the midpalatal suture. To assess the normality of variables, the Shapiro-Wilk test was performed. For intergroup comparison, ANOVA with a significance level of 5% was used. Results: At the region A, Hyrax-type (4.66 mm) and EDO (4.87 mm) groups presented larger openings than the Haas-type group (3.43 mm). In regions B and C, EDO showed a statistically significant greater opening than the Haas-type group. In region D, a smaller opening of the midpalatal suture was observed in the Haas-type group, compared to the Hyrax-type and EDO groups. Conclusions: EDO and Hyrax-type produced greater immediate skeletal effects, compared with Haas-type, but these differences were about 1 mm and might not be clinically significant.


RESUMO Objetivo: O objetivo do presente estudo foi testar a hipótese nula de que não existe diferença na abertura da sutura palatina mediana após a expansão rápida da maxila (ERM) usando os expansores Diferencial, tipo Hyrax e tipo Haas. Métodos: Radiografias oclusais de 52 pacientes (19 do sexo masculino e 33 do sexo feminino; idade média: 9,46 ? 1,20 anos) tratados com ERM foram divididas em três grupos, de acordo com o tipo de expansor usado: Diferencial (n = 17), tipo Hyrax (n = 21) e tipo Haas (n = 14). As variáveis avaliadas foram: A) distância entre os incisivos centrais superiores na borda incisal; B) distância entre os rebordos alveolares na sutura palatina mediana; C) abertura da sutura a 10 mm de distância da crista para posterior, na sutura palatina mediana; D) abertura da sutura a 20 mm da crista para posterior, na sutura palatina mediana; e E) abertura da sutura a 30 mm da crista para posterior, na sutura palatina mediana. Para verificar a normalidade das variáveis, utilizou-se o teste de Shapiro-Wilk. Para comparação intergrupos, usou-se a ANOVA com nível de significância de 5%. Resultados: Na região A, os grupos tipo Hyrax (4,66 mm) e Diferencial (4,87 mm) apresentaram uma abertura maior do que o grupo tipo Haas (3,43 mm). Nas regiões B e C, o Diferencial mostrou abertura significativamente maior do que o grupo tipo Haas. Na região D, uma abertura menor da sutura palatina mediana foi observada no grupo tipo Haas, comparado aos grupos tipo Hyrax e Diferencial. Conclusões: Os expansores Diferencial e tipo Hyrax produziram maiores efeitos esqueléticos imediatos, comparados ao tipo Haas, mas essas diferenças foram de aproximadamente 1 mm e podem não ser clinicamente significativas.

OBJECTIVE@#To assess three-dimensional (3D) changes of circummaxillary sutures following maxillary protraction with alternate rapid palatal expansions and constrictions (RPE/C) facemask protocol in maxillary retrusive children, and to investigate the relationship between the changes of circum-maxillary sutures and zygomaticomaxillary suture (ZMS) maturation, and to explore the factors of maxilla forward movement with RPE/C and facemask.@*METHODS@#In the study (clinical trial registration No: ChiCTR2000034909), 36 maxillary retrusive patients were recruited and block randomized to either the rapid palatal expansion (RPE) group or the RPE/C group. Patients aged 7 to 13 years, Class Ⅲ malocclusion, anterior crossbite, ANB less than 0°, Wits appraisal less than -2 mm, and A-Np less than 0 mm were included in the study. The RPE group received rapid palatal expansion, whereas the RPE/C group received alternate rapid palatal expansions and constrictions, and both with facemask protraction. Head orientations of cone-beam computed tomography (CBCT) images were implemented by Dolphin 11.7. 3D measurements of circummaxillary sutures on CBCT images were evaluated using Mimics 10.01 before (T0) and after treatment (T1). The changes were analyzed with independent t test, two-way ANOVA, Pearson correlation and regression analysis.@*RESULTS@#Two subjects in the RPE/C group were lost to follow-up. A total of 34 patients reached the completion criteria and were analyzed. Compared with the RPE group, sagittal changes of circummaxillary sutures were significantly increased in the RPE/C group with 1.21 mm advancement of zygomaticotemporal suture, 2.20 mm of ZMS, 1.43 mm of zygoma-ticofrontal suture (P < 0.05, respectively). Except for the zygomaticotemporal suture, the rest forward sagittal changes of other circummaxillary sutures showed no major difference in terms of the ZMS maturation. The Spearman's correlation in RPE/C indicated a strong positive correlation of sagittal changes between ZMS and point A (P < 0.01) with a regression analysis R2=42.5%.@*CONCLUSION@#RPE/C might be more effective on the treatment of maxillary retrusive children. As one of the major mechanical loading sutures during orthopedic therapy, ZMS showed a strong positive correlation with point A on sagittal changes.

Abstract Introduction The association between the treatment of transverse maxillary deficiency and the recovery of hearing and voice functions has gained attention in recent years. Objective This prospective controlled trial aimed to evaluate the effects of rapid maxillary expansion on hearing and voice function in children with non-cleft lip palate and bilateral cleft lip palate with transverse maxillary deficiency Methods 53 patients (26 non-cleft and 27 bilateral cleft lip palate; mean age, 11.1 ± 1.8 years) requiring rapid maxillary expansion for correction of narrow maxillary arches were recruited for this trial. Eight sub-groups were established based on the degree of hearing loss. Pure-tone audiometric and tympanometric records were taken for each subject at four different time periods. The first records were taken before rapid maxillary expansion (T0), the second after expansion (T1) (mean, 0.8 months), the third after three months (T2) (mean, 3 months) and the fourth at the end of retention period (T3) (mean, 6 months). ANOVA and Tukey HSD post-hoc tests were used for data analysis. Additionally, voice analysis was done using an updated PRAAT software program in a computerized speech lab at T0 and T2. A paired-samplet-test was used for comparisons of mean values of T0 and T2 voice parameters within both groups. Results Rapid maxillary expansion treatment produced a significant increase in the hearing levels and middle ear volumes of all non-cleft and bilateral cleft lip palate patients with normal hearing levels and with mild conductive hearing loss, during the T0-T1, T1-T2, T0-T2, and T0-T3 observation periods (p < 0.05). The significant increase was observed in right middle ear volumes during the T0-T1, T0-T2 and T0-T3 periods in non-cleft patients with moderate hearing loss. For voice analysis, significant differences were observed only between the T0 and T2 mean fundamental frequency (F0) and jitter percentage (p < 0.05) in the non-cleft group. In the cleft group, no significant differences were observed for any voice parameter between the T0 and T2 periods. Conclusion Correction of the palatal anatomy by rapid maxillary expansion therapy has a beneficial effect on both improvements in hearing and normal function of the middle ear in both non-cleft and bilateral cleft lip palate patients. Similarly, rapid maxillary expansion significantly influences voice quality in non-cleft patients, with no significant effect in BCLP patients.


Resumo Introdução A associação entre o tratamento da deficiência maxilar transversa e a recuperação das funções auditivas e vocais ganhou atenção nos últimos anos. Objetivo Avaliar os efeitos da expansão rápida da maxila na função auditiva e vocal em crianças sem fissura labiopalatina e com fissura labiopalatina bilateral com deficiência maxilar transversa. Método Foram recrutados para este estudo 53 pacientes (26 sem fissura e 27 com fissura labiopalatina bilateral; média de 11,1 ± 1,8 anos) que necessitam de expansão rápida da maxila para correção de arcos maxilares estreitos. Oito subgrupos foram estabelecidos com base no grau de perda auditiva. Registros audiométricos e timpanométricos de tons puros foram obtidos para cada indivíduo em quatro períodos. Os primeiros registros foram obtidos antes da expansão rápida da maxila (T0), o segundo após a expansão (T1) (média de 0,8 meses), o terceiro após três meses (T2) (média de 3 meses) e o quarto no fim do período de retenção (T3) (média de 6 meses). Anova e o teste post-hoc de Tukey HSD foram usados para análise dos dados. Além disso, a análise da voz foi feita com um programa PRAAT atualizado em um laboratório de fala computadorizadaem T0 e T2. Foi usado um teste t de amostras pareadas para comparação dos valores médios dos parâmetros de voz em T0 e T2 nos dois grupos. Resultados O tratamento com expansão rápida da maxila produziu um aumento significativo nos níveis auditivos e nos volumes da orelha média de todos os pacientes sem fissura e pacientes com fissura labiopalatina bilateral e níveis auditivos normais e com perda auditiva condutiva leve, durante os períodos de observação T0-T1, T1-T2, T0-T2 e T0-T3 (p < 0,05). Aumento significativo foi observado nos volumes da orelha média direita durante os períodos T0-T1, T0-T2 e T0-T3 em pacientes sem fissura e com perda auditiva moderada. Para a análise de voz, diferenças significantes foram observadas apenas entre a frequência fundamental média T0 e T2 (F0) e a porcentagem de jitter (p < 0,05) no grupo sem fissura. No grupo com fissura, não foram observadas diferenças significantes para nenhum parâmetro de voz entre os períodos T0 e T2. Conclusão A correção da anatomia palatal pela expansão rápida da maxila tem um efeito benéfico tanto na melhoria da audição quanto na função normal da orelha média em pacientes sem fissura e com fissura labiopalatina bilateral. Da mesma forma, a expansão rápida da maxila influencia significativamente a qualidade da voz em pacientes sem fissura, sem efeito significativo em pacientes com fissura labiopalatina bilateral.

ABSTRACT Introduction: Due to the anatomical constraints of the mandible, mandibular dental arch usually serves as a guideline to determine the required changes in the maxillary transverse dimension. The Schwarz appliance and the Lip Bumper are the traditional orthodontic appliances for mandibular arch expansion in patients with borderline amounts of crowding, and/or transverse discrepancy. However, they often require patient cooperation, which may be a concern for orthodontists in daily practice. Objectives: This article illustrates a simple fixed orthodontic device as an alternative to achieve mandibular arch expansion in patients with moderate tooth-size/arch-length discrepancy. The four reported cases refer to 8 to 10-year-old patients in the mixed dentition, with an Angle Class I or Class II malocclusion, transverse deficiency in both arches, moderate crowding and/or posterior crossbite, combined with compromised smile aesthetics. The patients were treated with rapid maxillary expansion (RME) using Hass expander appliance and the modified Arnold expander (MAE). Conclusion: This low-cost compliance-free orthodontic appliance provided dentoalveolar decompensation by means of uprighting the posterior teeth, with minimal or no adjustments during treatment. The final results were achieved in only three to four months, and fulfilled all treatment objectives, such as an increase in the arch perimeter and width, and a better teeth alignment.


RESUMO Introdução: Devido aos limites anatômicos da mandíbula, a arcada dentária inferior geralmente serve como guia para determinar as alterações necessárias na dimensão transversal da maxila. O aparelho de Schwarz e o Lip Bumper são os aparelhos usados tradicionalmente para expansão da arcada inferior em pacientes com quantidades limítrofes de apinhamento e/ou discrepância transversal. No entanto, eles requerem a cooperação do paciente, o que pode ser uma preocupação para os ortodontistas na prática diária. Objetivos: O presente artigo ilustra uma alternativa diferente de aparelho fixo para se obter a expansão da arcada inferior em pacientes com discrepância moderada de tamanho dentário e/ou comprimento da arcada. Os quatro casos relatados referem-se a pacientes com 8 a 10 anos de idade, na dentição mista, com má oclusão de Classe I ou II de Angle, deficiência transversal em ambas as arcadas, apinhamento moderado e/ou mordida cruzada posterior, apresentando comprometimento da estética do sorriso. Os pacientes foram tratados com expansão rápida da maxila (ERM), usando aparelho expansor de Hass, e expansor Arnold modificado (EAM). Conclusão: O EAM, que é um aparelho de baixo custo e não depende da colaboração do paciente, promoveu uma descompensação dentoalveolar por meio da verticalização dos dentes posteriores, necessitando de mínimo ou nenhum ajuste durante o tratamento. Os resultados pretendidos foram alcançados em três a quatro meses e cumpriram todos os objetivos do tratamento, como aumento do perímetro e largura da arcada, assim como o melhor alinhamento dos dentes.

ABSTRACT Objective: To assess the impact of wearing palatal expanders on the oral health-related quality of life (OHRQoL) of children aged 8 to 10 years. Material and Methods: The sample consisted of 35 children aged 8 to 10 years treated at the orthodontics and pediatric dentistry outpatient clinics affiliated with the Dental School. Children were divided in two groups: 17 were submitted to palatal expansion treatment (Group 1), while 18 just received coronal polishing and topical fluoride application (Group 2). The Brazilian version of the Child Perceptions Questionnaire (CPQ8-10) was used to assess children's OHRQoL. The questionnaire was administered before the expander was placed, at the end of its activation, and before its removal. Intervals between the interviews were similar in the two groups. Descriptive statistics and ANOVA were used for data analyses. Results: The overall CPQ8-10 score was not significantly different between the groups. The results show statistically significant differences between the two groups only for the functional limitations domain after activation of the appliance (p=0.001). Conclusion: Palatal expanders may negatively affect children's functions only during the initial wearing period.

ABSTRACT Introduction: In Orthodontics and Facial Orthopedics, the timing of treatment onset may be critical and individual analysis should be applied to promote a favorable treatment planning. In this study, individual analysis of midpalatal suture (MS) and palatal measurements were performed in teenagers and young adult patients treated with rapid maxillary expansion (RME). Description: Twenty-six patients submitted to RME with a tooth-supported appliance (Hyrax) were evaluated. The inclusion criteria were: minimum age of 14 years, presenting all posterior teeth, diagnosed with transverse maxillary discrepancy, and with a clinical indication for maxillary expansion. The pretreatment CBCT scans of these patients were assessed to obtain the stages of MS maturation (MSM); density ratio (MSD); and palatal length, thickness (anterior, intermediate and posterior) and sagittal area. Results: The maturation stages present were C, D or E; the density ranged from 0.6 to 1, and lower density (MSD < 0.75) and higher density (MSD ≥ 0.75) groups were determined. Individuals with higher MSD presented smaller sagittal area, compared to the lower density group. Individuals in D and E MSM stages presented smaller sagittal area and intermediate thickness, compared to stage C. Conclusions: Smaller palatal sagittal area was observed in the high MSD groups and in the stages D and E of MSM.


RESUMO Introdução: Em Ortodontia e Ortopedia Facial, o momento de início do tratamento pode ser crítico, e uma análise individual deve ser aplicada para promover um planejamento de tratamento favorável. No presente estudo, foram realizadas a avaliação individualizada da sutura palatina mediana (SPM) e medições no palato de adolescentes e adultos jovens tratados com expansão rápida da maxila (ERM). Descrição: Foram avaliados vinte e seis pacientes submetidos à ERM com aparelho dentossuportado (Hyrax). Os critérios de inclusão foram: idade mínima de 14 anos, apresentando todos os dentes posteriores, diagnosticado com discrepância transversa da maxila e com uma indicação clínica para expansão maxilar. A tomografia computadorizada de feixe cônico (TCFC) pré-tratamento desses pacientes foi avaliada para obter os estágios de maturação da SPM (MSPM), densidade da SPM (DSPM), comprimento do palato, espessura (anterior, intermediária e posterior) e área sagital. Resultados: Os estágios de maturação presentes foram C, D ou E; a densidade variou de 0,6 a 1, e foram determinados grupos de baixa (DSPM < 0,75) e alta densidade (DSPM ≥ 0,75). Indivíduos com maior DSPM apresentaram menor área sagital, em comparação com o grupo de densidade mais baixa. Indivíduos nos estágios D e E de MSPM apresentaram menor área sagital e espessura intermediária, comparados aos indivíduos no estágio C. Conclusão: Uma menor área sagital palatina foi observada nos grupos de alta DSPM e nos estágios D e E de MSPM.

ABSTRACT Introduction: Skeletal posterior crossbite (SPCB) has a multifactorial etiology, as it may be caused by parafunctional habits, atypical position of the tongue, tooth losses and maxillary or mandibular transverse skeletal asymmetries. Skeletal involvement may lead to facial changes and an unfavorable aesthetic appearance. The treatment of SPCB diagnosed in an adult patient should be correctly approached after the identification of its etiologic factor. Surgically-assisted rapid maxillary expansion (SARME), one of the techniques used to correct SPCB in skeletally mature individuals, is an efficient and stable procedure for the correction of transverse discrepancies that may be performed in the office or in a hospital. Objective: This study discusses the results of asymmetrical SARME used to correct unilateral SPCB associated with transverse mandibular asymmetry. Conclusion: The treatment alternative used in the reported case was quite effective. At the end of the treatment, the patient presented adequate occlusion and facial aesthetics.


RESUMO Introdução: A mordida cruzada posterior esquelética (MCPE) apresenta etiologia multifatorial, podendo ser causada por hábitos parafuncionais, posição atípica da língua, perdas dentárias e assimetrias esqueléticas transversais da maxila ou da mandíbula. Alterações faciais podem estar presentes quando há envolvimento esquelético, levando a estética desfavorável. O tratamento da MCPE, quando diagnosticada no paciente adulto, requer abordagem correta, com identificação do fator etiológico. Entre as técnicas utilizadas para correção da MCPE em pacientes esqueleticamente maduros, cita-se, em especial, a Expansão Rápida de Maxila Assistida Cirurgicamente (ERMAC). Essa modalidade tem se mostrado bastante eficiente na correção dos problemas transversais, apresenta estabilidade e pode ser realizada em ambiente ambulatorial ou hospitalar. Objetivo: O objetivo do presente trabalho será discutir os resultados da ERMAC assimétrica para correção da MCPE unilateral associada a assimetria transversal da mandíbula. Conclusão: A alternativa de tratamento utilizada no caso relatado mostrou-se bastante eficiente. Ao fim do tratamento, o paciente apresentou adequada oclusão e boa estética facial.

ABSTRACT Objective: Assess the influence of mono- and bicortical anchorage and diameter of mini-implants (MIs) on the primary stability of these devices. Methods: 60 self-drilling MIs were distributed in six groups according to diameter (1.5mm, 1.8mm or 2.0mm) and type of anchorage (monocortical and bicortical) in bovine rib. The primary stability was evaluated by insertion torque, micromobility and pull-out strength tests. ANOVA and/or Tukey analysis were used to conduct intergroup comparisons (p< 0.05). Non-parametric statistics (Kruskal-Wallis and Mann-Whitney) were performed when normality was not found (p< 0.05). Results: MIs with larger diameters and bicortical anchorage showed greater primary stability regarding insertion torque (p< 0.05) and micromobility (p< 0.05). Only MI diameter had an effect on the pull-out strength test. Larger diameter MIs presented better retention in pull-out strength tests (p< 0.001), regardless of mono- or bicortical anchorage. Conclusions: MI primary stability is dependent on its diameter and type of anchorage. Bicortical anchorage showed greater stability when compared with monocortical anchorage, independently of other variables.


RESUMO Objetivo: Avaliar a influência da ancoragem mono e bicortical e do diâmetro dos mini-implantes (MIs) na estabilidade primária desses dispositivos. Métodos: 60 MIs autoperfurantes foram distribuídos em seis grupos, de acordo com o diâmetro (1,5 mm, 1,8 mm ou 2,0 mm) e tipo de ancoragem (monocortical e bicortical), e inseridos em costela bovina. A estabilidade primária foi avaliada pelos testes de torque de inserção, micromobilidade e resistência à tração. ANOVA e/ou análise de Tukey foram usadas para realizar comparações intergrupos (p< 0,05). Estatística não paramétrica (Kruskal-Wallis e Mann-Whitney) foi realizada quando a normalidade não foi encontrada (p< 0,05). Resultados: MIs com diâmetros maiores e ancorados bicorticalmente apresentaram maior estabilidade primária em relação ao torque de inserção (p< 0,05) e micromobilidade (p< 0,05). Apenas o diâmetro do MI influenciou os resultados do teste de resistência à tração. MIs de maior diâmetro apresentaram melhor retenção nos testes de resistência à tração (p< 0,001), independentemente da ancoragem mono ou bicortical. Conclusões: a estabilidade primária do MI é dependente de seu diâmetro e tipo de ancoragem. A ancoragem bicortical apresentou maior estabilidade quando comparada à ancoragem monocortical, independentemente das demais variáveis.

ABSTRACT Objective: To compare the dento-alveolar effects between dental and skeletal anchored expansion devices to achieve maxillary expansion in different groups of patients: pure dental anchorage was used in growing patients and skeletal anchorage in adult patients. Linear, angular and volumetric parameters have been analysed. Material and Methods: 22 patients has been selected from the total archive of patients treated in the Orthodontics Department of the University of Ferrara, Italy, divided into 3 groups according to the appliance used to obtain the transversal maxillary correction. Digital files of the upper arch were obtained from scans performed using an intro-oral scanner before treatment (T0) and at the end of expansion (T1). Therefore, linear, angular and volumetric measurements have been performed. Results: There is a highly significant statistic variation of the 3-3, 4-4 and 6-6 diameter as a function of time. There was a statistically significant variation of the palatal vault angle and of the 6-6 distance at the cusp level as a function of time. Moreover, it appears that these parameters are reduced in the group with skeletal anchored expander. No tooth considered have encountered a statistically significant change in tip and torque values, no matter the type of RPE and the treatment time. For all patients, we have recorded the superimposition of the maxillary dental cast before and after treatment. Conclusion: In all patients, we obtained a resolution of the maxillary transverse deficit. Patients treated by pure skeletal anchored devices have recorded a less variations of palatal vault angle and the distance between first molar cusps, as if the expansion was gained more parallel. The expansion obtained showed reduced dental side effects, as tip and torque values haven't changed in a statistically significant way.

ABSTRACT Objective: To evaluate how the lower arch spontaneously change after upper rapid palatal expansion in a group of patients with transversal skeletal deficit. Material and Methods: Twenty-four patients treated by the same orthodontist with a rapid palatal expander (RPE) bounded on a deciduous molar have been selected. The sample was divided into two groups: no treatment was provided for group one, while group two was treated using a lip bumper or Schwarz appliance. For each patient, dental casts were collected when the RPE was bounded (T0) and at the end of treatment, 9 months ± 3 months later (T1). Each outcome was analyzed, providing descriptive statistics, main effects significance tests and post-hoc analyses with the objective to evaluate the variations between pre-treatment (TO) and post-treatment (T1) of each of them. Results: If the linear measurements are considered, a significant beneficial effect on both arches is observed. However, the upper arch always shows a major increase of all values at T1 with respect to the lower arch. Even though the post-hoc tables indicate that time differences are all statistically significant across considered partitions, the lower arch's increase is more pronounced in group two, where patients were treated in both arches. If the angular measurements are concerned, the increase of lingual crown inclination was found in all patients, independently from the type of treatment in lower arch. Conclusion: All patients show normalization of upper diameters, regardless of whether the lower arch was treated or not.

ABSTRACT Transverse deficiencies should be a priority in orthodontic treatment, and should be corrected as soon as diagnosed, to restore the correct transverse relationship between maxilla and mandible and, consequently, normal maxillary growth. Corrections may be performed at the skeletal level, by opening the midpalatal suture, or by dentoalveolar expansion. The choice of a treatment alternative depends on certain factors, such as age, sex, degree of maxillary hypoplasia and maturation of the midpalatal suture. Thus, the present study discusses different treatment approaches to correct maxillary hypoplasia in patients with advanced skeletal maturation.


RESUMO Os problemas transversais devem ser priorizados no tratamento ortodôntico e corrigidos assim que diagnosticados, para restituir a correta relação transversal entre maxila e mandíbula e, consequentemente, restabelecer o crescimento maxilar normal. A correção pode ser realizada em nível esquelético, por meio da abertura da sutura palatina mediana (SPM), ou por expansão dentoalveolar. A opção de tratamento depende de alguns fatores como idade, sexo, grau de hipoplasia da maxila e a maturação da SPM. Assim, o objetivo do presente trabalho foi discutir as diferentes abordagens terapêuticas para correção da hipoplasia maxilar em pacientes com maturação esquelética avançada.

@#Transverse maxillary deficiency is a common malocclusion in the clinic. Palatal expansion techniques are commonly used in the treatment of maxillary transverse deficiency. Traditional palatal expansion techniques have good effects on the treatment of children and adolescents whose palatal suture has not yet closed, but the effects on adult patients are unsatisfied. New palatal expansion techniques, such as miniscrew-assisted rapid palatal expansion and surgical-assisted maxillary expansion, have increased the age-related indications for palatal expansion, and their bone expansion effect has been strengthened. With the development of CAD/CAM technology and 3D printing technology, techniques such as Invisalign and personalized appliances have been developed and have promising application prospects. To provide references for the clinical treatment of maxillary transverse deficiency, palatal expansion techniques are reviewed from the following aspects: mechanism and indications, expansion pattern, traditional and new palatal expansion appliances, stability and retention of palatal expansion, outlook of future expanders, etc.

ABSTRACT Objective: The aim of this study was to estimate the changes in the palate area after rapid maxillary expansion (RME) with the Hyrax expander in growing subjects, using cone beam computed tomography (CBCT). Methods: Fourteen patients (9 girls and 5 boys; mean age = 11.7 ± 2.4 years) who required RME as part of their orthodontic treatment were included in this study. CBCT records had been taken before RME treatment (T0), at the end of active expansion (T1) and after a 6-month retention period (T2). The CBCT scans were manipulated with Dolphin Imaging® version 11.7 Premium software, in which landmarks were positioned and measured in relation to sagittal, coronal and axial planes, to verify the palate surface area. In addition, linear measurements of the palatal depth and width were assessed. These measurements were compared by using analysis of variance (ANOVA) for repeated measures. A p-value smaller than 0.05 was considered statistically significant. Results: The palatal surface area and width significantly increased from T0 to T1, respectively by 9.27% and 9.71%, and both decreased in a non-significant manner from T1 to T2. The palatal depth had non-significant differences at T0, T1 and T2. Conclusions: RME promotes a significant gain in the surface area of the palate and an increase in intermolar width. The Hyrax appliance was effective for the treatment of maxillary atresia in growing patients. There was no vertical alteration of the palate. After a 6-month retention period, the maxilla transverse dimension and the surface area of the palate remained stable.


RESUMO Objetivo: o objetivo do presente estudo foi estimar a alteração na área do palato após expansão rápida da maxila (ERM) com o expansor Hyrax em indivíduos em crescimento, utilizando tomografia computadorizada de feixe cônico (TCFC). Métodos: quatorze pacientes (9 meninas e 5 meninos; idade média 11,7± 2,4 anos) que necessitavam de ERM como parte de seu tratamento ortodôntico foram incluídos nesse estudo. Os registros de TCFC foram obtidos antes do tratamento com ERM (T0), ao término da expansão ativa (T1) e após um período de contenção de 6 meses (T2). Os exames de TCFC foram manipulados no software Dolphin Imaging® versão 11.7 Premium, no qual os pontos anatômicos foram posicionados e medidos em relação aos planos sagital, coronal e axial, para verificar-se a área de superfície do palato. Além disso, medidas lineares da profundidade e largura do palato foram avaliadas. Essas medidas foram comparadas por análise de variância (ANOVA) para medidas repetidas. Valores de p< 0,05 foram considerados estatisticamente significativos. Resultados: a área de superfície e a largura do palato aumentaram significativamente de T0 para T1, respectivamente em 9,27% e 9,71%, e ambas diminuíram de forma não significativa de T1 para T2. A profundidade do palato teve diferenças não significativas em T0, T1 e T2. Conclusões: a ERM promove ganho significativo na área de superfície do palato e aumento na largura intermolar. O aparelho Hyrax foi efetivo para o tratamento da atrasia maxilar em pacientes em crescimento. Não houve alteração vertical no palato. Após um período de contenção de 6 meses, a dimensão transversal da maxila e a área de superfície do palato permaneceram estáveis.

Introduction: Maxillary constrictions are one of the most frequent abnormalities, regardless of the type of dentition studied. They can be treated either with tooth-borne or bone-borne expander appliances, depending on the biological maturation of the patient. Treatment during prepubertal stages has been shown to be successful, but there is evidence of an open suture even in late adolescence stages. The aim of this study is to compare the effects of a rapid expansion bone-borne device to a tooth-borne device in young adults suffering from maxillary constriction, by means of Cone-Beam Computed Tomography. Materials and methods: A retrospective observational study was conducted in nine patients (three males, six females) with a mean age of 18±5.5 years, who attended the Dentistry Service of Universidad de Chile during 2014-2015. Six patients were treated with tooth-borne appliances (Hyrax type) and three patients with bone-borne appliances (MARPE). Linear measurements of the anterior and posterior transverse dimension and angular measurements of the inclination of molars and incisors were obtained through CBCT for each group in T1 and T2. Results: A significant increase in the posterior transverse dimensions in both groups was observed. Radiolucency of the suture was observed in all patients treated with miniscrews and in half of the patients treated with Hyrax. Greater angulation of molars was shown in the tooth-borne group in comparison with the bone-borne group. Conclusion: Both appliances caused an increase in the posterior transverse dimension, but bone anchored appliances achieved a more parallel and skeletal expansion. Randomized clinical trials with a larger sample and follow-up are needed.


Introducción: Las compresiones maxilares son una de las anomalías más frecuentes, independiente del tipo de dentición que se esté estudiando. Su tratamiento se puede realizar con un aparato expansor ya sea dentosoportado o esqueletalmente soportado, dependiendo de las consideraciones de maduración biológica del paciente. Se ha demostrado éxito en el tratamiento en etapas prepuberales, sin embargo, existe evidencia de una sutura abierta aún en etapas post adolescencia. El objetivo de este estudio es comparar, mediante Cone Beam CT, los efectos de un dispositivo de expansión rápida maxilar esqueletamente soportado, en relación a los dentosoportados, en adultos jóvenes con compresión maxilar. Material y método: Se realizó un estudio de tipo observacional retrospectivo, donde se evaluaron nueve pacientes (tres hombres y seis mujeres) con un promedio de edad de 18±5,5 años,que asistieron al Servicio de Ortodoncia de la Universidad de Chile, durante los años 2014-2015. Seis pacientes fueron tratados con aparatos dentalmente soportados (tipo Hyrax) y tres pacientes con apartos esqueletalmente soportados (MARPE). A través de CBCT se tomaron medidas lineares de la dimensión transversal anterior y posterior, y medidas angulares de la inclinación de molares e incisivos para cada grupo en T1 y T2. Resultados: Hubo un aumento significativo en la dimensión transversal posterior de ambos grupos. Se presentó radiolucidez de la sutura en un 100% de los pacientes tratados con microtornillos y 50% de los pacientes tratados con hyrax. Hubo mayor angulación de molares en el grupo dentosoportado en comparación al grupo esqueletalmente soportado. Conclusiones: Ambos aparatos lograron un aumento de la dimensión transversal posterior, sin embargo, los de anclaje esqueletal lograron una expansión más paralela y esqueletal. Hacen falta ensayos clínicos aleatorizados con mayor muestra y seguimiento.

Abstract Introduction The rapid maxillary expansion (RME) reduces the risk of developing structural and functional disorders in the stomatognathic system. Objective To examine the effects of the RME as a treatment for the posterior crossbite, related with the electromyographic activity of the masticatory muscles and the TMJ noises in a population of children. Material and method 13 girls and 7 boys, regardless of the type of malocclusion, with a mean age of 9 years old (± 3), were treated with RME. The electrovibratography analyzed the TMJ noise, and the electromyography analyzed the masticatory muscles before treatment (T0) and after three months of a short-term follow-up (T1). The comparisons of the affected and unaffected sides by the crossbite were performed using Mann-Whitney's test, and to compare data before and after treatment the Wilcoxon's test was used (level of significance: 5%). Result No significant differences were found in the parameters of joint noise in comparison to the sides affected and unaffected by the crossbite, in both T0 and T1 (p>0.05); only the side without the crossbite observed decrease in the peak amplitude of the joint noises after treatment. In the static electromyographic analysis, inter-side differences were observed before and after treatment, since the deliberate unilateral chewing showed greater asymmetry activity in T0 for both sides, which has been corrected after treatment, improving the functional chewing. Conclusion The proposed treatment did not lead to the occurrence of joint noises and improved the functional pattern of electromyographic activity during chewing at the end of treatment.


Resumo Introdução A expansão rápida da maxila (RME) reduz o risco de desenvolvimento de distúrbios estruturais e funcionais no sistema estomatognático. Objetivo Analisar os efeitos desta intervenção como tratamento para a mordida cruzada posterior, relacionados à ocorrência de ruídos nas articulações temporomandibulares e à atividade eletromiográfica dos músculos masseter e temporal anterior. Material e método 13 meninas e 7 meninos, independentemente do tipo de maloclusão, com idade média de 9 anos (±3), foram tratadas com RME. Por meio de eletrovibratografia analisou-se ruídos nas articulações temporomandibulares, e de eletromiografia de superfície a atividade dos músculos mastigatórios antes (T0) e após 3 meses do final do tratamento proposto (T1). As comparações entre os lados afetado e não afetado pela mordida cruzada foram realizadas utilizando-se o teste de Mann-Withney. As comparações de antes e após o tratamento foram realizadas pelo teste de Wilcoxon (nível de significância: 5%). Resultado Não houve diferença significativa na eletrovibratografia entre os lados afetado e não afetado pela mordida cruzada, tanto em T0 como em T1 (p>0.05); do lado sem mordida cruzada observou-se diminuição do pico de amplitude dos ruídos articulares após a expansão rápida da maxila (p<0.05). Na análise da eletromiografia estática foram observadas diferenças inter-lados antes e após o tratamento, uma vez que a mastigação deliberada unilateral apresentou maior atividade de assimetria em T0 para ambos os lados, o que foi corrigido após o tratamento (p<0.05), melhorando a mastigação funcional padrão. Conclusão O tratamento proposto para mordida cruzada posterior funcional não levou à ocorrência de ruídos articulares e melhorou o padrão funcional da atividade eletromiográfica durante a mastigação ao final do tratamento.
